In light of that, I would like to share with you something that I have utilized for a number of years with significant success in reducing current caries and periodontal disease.
Topical fluoride applied to the toothbrush or as toothpaste has a minimal significance for our adult population. Having observed that over the years, I have gone back to an old modality with great success. Custom made fluoride trays, using Prevident 5000 and worn by the patient at least an hour per day and particularly at night, can significantly impact the caries rate and reduce plaque. Fluoride used over a longer period of time, when held closely against the gingival tissue, can markedly diminish the accumulated soft debris and cariogenic plaque. The fabrication of these trays is no different than the way we would fabricate bleaching trays. I urge you to try them on your patients who have recurrent decay or who have difficulty controlling plaque because of a lack of dexterity or will.
